Tubal ligation reversal is a surgical procedure to reconnect fallopian tubes that were previously closed or blocked with a tubal ligation. If you decide that you’d like the chance to conceive again, tubal ligation reversal gives you the opportunity to restore your fertility.

Tubal ligation reversal is performed under general anaesthesia, meaning you’ll be asleep during the procedure. Depending on the type of tubal ligation you had, your surgeon may reconnect your fallopian tubes by removing any scar tissue, cauterised ends or other blockages that were created during the original procedure. 

It may be performed as keyhole (laparoscopic) surgery or as open surgery: 
  • For open surgery, your surgeon will make a small incision in your abdomen to access your fallopian tubes
  • For keyhole surgery, they’ll gain access using a few very small incisions in your abdomen through which they’ll insert a telescope and thin surgical instruments
In some cases, your surgeon may suggest using robotic-assisted surgery. They’ll discuss the benefits of various approaches with you in detail so you can reach the best decision, together.

How to prepare

If we agree that reversal surgery is right for you, you’ll get all the help you need to prepare. We’ll carry out pre-operative assessments such as blood tests and imaging to evaluate your reproductive health and discuss an anaesthesia plan, to help keep you comfortable during the procedure. 

You’ll receive clear guidelines on fasting before surgery and information about any medications that you should avoid.

Your surgery

You’ll be given a general anaesthetic so you’re asleep during the procedure. Your surgeon will carefully reconnect your fallopian tubes using laparoscopic techniques or open surgery. Robotic-assisted surgery may be an option, depending on your surgeon’s preferences and approach. 

The procedure usually takes about two hours.

Aftercare

If you’ve had a keyhole operation, you should be able to go home the same day although you won’t be able to drive so you’ll need someone to take you home. You’ll be given detailed aftercare instructions to help you have a smooth and successful recovery. 

For open surgery you’ll need to stay overnight, until your team is happy with your recovery.

Over-the-counter pain relief will usually be enough to help you manage any discomfort, and you should avoid strenuous activities. We'll schedule check-ups to monitor your recovery and discuss next steps on your fertility journey.

As with any surgery, tubal ligation reversal carries risks. While most procedures go smoothly, it's important to understand any potential complications so you can make an informed decision. 

Surgical risks include:

  • Infection
  • Bleeding or blood clots
  • Reactions to anaesthesia
  • Scar tissue forming around the surgical site
  • Damage to other organs during surgery

Having this procedure can also increase your risk of having a subsequent ectopic pregnancy. With an ecotopic pregnancy, a fertilised egg implants outside of your womb, and this can be life-threatening if it’s not treated promptly. So if you do get pregnant, it’s vital that your early months are closely monitored.

Every woman's body is different, but most pregnancies happen within the first year or two following a successful reversal surgery. You'll need to wait two menstrual cycles before trying to conceive. 

If your tubes are successfully reconnected and there are no other fertility concerns, many women can conceive naturally after reversal. However, if you haven’t successfully conceived within two years, your doctor may recommend further testing or fertility support.
This can be a highly successful surgery, for the right women. 50-80% become pregnant within the first two years after having the procedure, although those success rates tend to be lower if you’re over 40 years old.

The best way to gauge your chances is by having a fertility consultation. This is chance to fully assess your history and current reproductive health with a specialist. If reversal isn't the best option, your specialist will talk you through alternatives.

The most common alternative is in vitro fertilisation (IVF) or test-tube baby treatment. This procedure bypasses your fallopian tubes entirely by fertilising your eggs in a lab and transferring the embryos directly into your womb. IVF may be a better choice if:

  • Your fallopian tubes are too damaged or too short
  • You're over 40 years old and time is a factor
  • You'd prefer not to undergo surgery
  • Your partner has fertility issues

IVF has a high pregnancy rate and doesn't involve any surgery. However, it does require many complex steps. A fertility specialist will take the time to explore what's right for your personal goals and circumstances.
