Nicholas qualified from the University of Birmingham Medical School in 1991 and trained in infectious diseases and general (internal) medicine in London.

Nicholas became a consultant in 2005 and was appointed as director of the infection and director of infection prevention and control in 2009. He has led the national High Consequence Infectious Diseases (Airborne) Network since 2018. Other specialist clinical interests include tuberculosis and vascular graft infection.
