Surgical Expertise

Severe Deep Infiltrating Endometriosis


Our multidisciplinary endometriosis team offers top surgical expertise in excisional nerve sparing, radical and fertility preserving treatment options of severe infiltrating endometriosis. Our aim is to avoid multiple surgical operations for deep endometriosis and clear the disease in a single surgical procedure, even if the disease affects multiple organs and systems. We use principles of “conservative radicality” and nerve sparing and our target is to excise the disease in full and cause minimal collateral damage to ovarian reserve, other pelvic organs, nerves and blood vessels.


Bowel Endometriosis 


We use innovative surgical techniques for excision of deep bowel endometriosis. For suspected deep infiltrating endometriosis of the bowel we offer extensive pre-operative assessment including special MRI scan to assess the extent of the disease before surgery. If deep infiltrating endometriosis of the bowel is confirmed by the MRI we offer surgical treatment options which may include bowel shaving, discoid stapler bowel resection or, for large or multiple bowel endometriosis nodules - economical segmental bowel resection. All these procedures are performed laparoscopically. Stapler discoid bowel resection is the most innovative technique of excising deep infiltrating endometriosis of the bowel without removing the length of bowel. This technique offers lower complications rate, low recurrence rate and quicker recovery. The Harley Street Clinic Endometriosis Centre is one of the first specialist units in the UK to widely use this technique for suitable patients with bowel endometriosis.  Our surgical audit data shows excellent results in improving patients quality of life, pain levels and sexual function. Our serious complications rate for bowel endometriosis surgery is very low and we take enormous efforts to maintain patients wellbeing before, during and after surgery.


Urinary Tract Endometriosis


If bladder endometriosis is confirmed by the MRI, we offer excisional treatment which may include full thickness bladder nodules excision, bladder reconstruction and ureteric re-implantation. All these procedures are performed laparoscopically.


Diaphragmatic Endometriosis


We offer excisional treatment for confirmed diaphragmatic endometriosis. These operations are performed by a joint surgical team involving gynaecology, upper GI and thoracic surgeons. Procedures are performed endoscopically.

    Our patients consultations with endometriosis consultants are friendly, meticulous and very detailed. We aim to reach diagnosis of endometriosis without delay, using clinical expertise of our consultants, latest imaging techniques (high resolution ultrasound and MRI) and state of the art endoscopy equipment. We will provide treatment, aiming to remove all visible endometriosis, and afterwards offer our patients options to manage endometriosis long term, taking into consideration individual circumstances, fertility aspirations and predominant symptoms.

    To make a diagnosis of endometriosis, we carry out a detailed clinical assessment of patient’s symptoms, gynaecological examination by a consultant gynaecologist, high resolution ultrasound and MRI scans of pelvis and abdomen and possibly a diagnostic laparoscopy, the gold standard diagnostic procedure, which involves inserting an ultra-high definition endoscopic camera into the pelvis through a small cut in the umbilicus under general anaesthetic. Depending on the severity of endometriosis, further investigations may focus on the bladder, bowel and other organs which might be affected by disease. If fertility is required, full assessment of ovarian hormonal function, tubal patency and ovarian reserve is offered alongside with other investigations.

    The multidisciplinary team approach to surgical treatment allows us to perform the most complex surgical operations for endometriosis, keeping the complications to the absolute minimum. The treatment for endometriosis does not stop after surgery and we offer continuous care and support to all our patients, depending on their symptoms and needs. This includes multidisciplinary team care including management of chronic pain, bowel symptoms and infertility.

    In surgical treatment of endometriosis, we use the most advanced surgical techniques and equipment available in the modern world. To excise deep endometriosis, we use ultrasonic energy, rotating bipolar forceps and cold scissors. We aim to achieve full clearance of endometriosis and not leave any disease behind, however complex the surgery might be. We practice nerve sparing laparoscopic surgical technique and principle of "economical radicality" in excision of endometriosis nodules. In cases of most severe endometriosis we always work as a multidisciplinary team of gynaecologist, urologist and colorectal surgeon in order to achieve disease clearance in one operation and avoid multiple surgical procedures.

     Endometriosis can severely affect the quality of life. Depression and chronic tiredness are very common symptoms of endometriosis. To help our patients fight the disease we offer support of Clinical Psychologists, Pain Specialists and Nutritionists. Our specialist endometriosis nurse is accessible on a constant basis and serves as a link between a patient and specialist consultants providing endometriosis care.
