Mr Machesney would like to reassure his privately insured patients that his charges will comply within their insurer's guidelines.

Mr Machesney is the Chair of the newly constituted Colorectal Cancer Clinical Reference Group (CRG) for NHS England with an ambition to improve the patient experience, safety and early diagnosis. He is also the Pathway Director for

Colorectal Cancer at London Cancer, an integrated cancer system (ICS) resulting from the amalgamation of the North East London Cancer Network (NELCN) and the North Central London and West Essex Cancer Network (NCLWECN) following the Cancer Review for London (CRL). Before the formation of the ICS he was Chair of the Colorectal Tumour Advisory Board for the NELCN. He was also the Clinical Director for Surgery, Critical Care and Anaesthesia at Whipps Cross Hospital up to the time of the merger with Newham General Hospital and Bart’s and the Royal London Hospitals, to form Bart’s Health NHS Trust. He is involved with National Peer Review and has been a CQC Hospital Inspector. He is on the Clinical Advisory Board of Bowel Cancer UK.

He has been in consultant practice as a surgeon since 2003 and for the first nine years of his appointment was the Lead Clinician for Colorectal Cancer at Whipps Cross University Hospital NHS Trust. During the time as Lead Clinician, he built a team that was an early adopter of laparoscopic surgery and enhanced recovery for colorectal patients. Whipps Cross was recorded in the CRL Case for Change document (2008) to have had the second highest rate for laparoscopic surgery and the lowest length of stay for colectomy in the Capital.

As Pathway Director at London Cancer, working with his colleagues at UCL Partners, he has built a Board and developed relationships with Commissioning Support, Primary Care Commissioners, Colorectal Charities, Patient Groups, Clinicians and his colleagues in the other London ICS, the London Cancer Alliance. He is a member of the NHS England (London) Early Detection and Awareness Group where he was co-author of New London Referral Guidelines to promote early diagnosis of bowel cancer by lowering the threshold for GPs to refer. He commissioned and participated in the implementation of a triage straight to test pathway for patients with colorectal symptoms at Whipps Cross, which was shortlisted for a BMJ Award in 2014.
